In order to be considered for employment, all forms must be filled out completely, signed, and submitted to our office. You can manually fill out the form or you can fill out the form on your mobile device or computer and digitally sign. Completed, signed forms must be submitted to our office by any of the following methods: email to firstname.lastname@example.org, mail or deliver to 105 E Street, Suite 400, Hampton VA 23661, or fax to 757-873-3010. Completed applications will be considered for a period of up to 60 days from the date submitted. You may renew your application once every 60 days for up to a year by calling, visiting our office, mail, email or fax. Applications on file will be destroyed after one year from the date originally submitted.
